The common symptom associated with a bruised kidney is pain specifically on the sides of the abdomen and in the flank area. This is the region between the lower rib cage and upper hip. Small fluid collection surrounds the lower pole of the left kidney with associated perinephric stranding and thickening of the posterior and to a lesser extent the anterior pararenal fascia. This most likely represents a ruptured left renal cyst. Left psoas hematoma and minor retroperitoneal hematoma - no pancreatic duodenal injury noted. No rib or sternal fracture. CT Abdomen Pelvis. There is no perfusion of the left kidney. The left renal artery demonstrates normal opacification within its proximal 7 mm. There is however immediate truncation of the artery at a point of retroperitoneal hematoma. This hematoma lies immediately posterior to a laceration of the body of the pancreas. The kidney is injured in up to 10 of patients who sustain significant abdominal trauma. Overall about 65 of genitourinary GU injuries involve the kidney. The phrase chronic pyelonephritis CP is not really one diagnosis. It is meant to mean the long-term damage done by recurrent urine infection to the drainage system of the kidney. But it has come to be a fallback diagnosis i.e. when doctors are not sure of the cause of kidney failure and the kidney has scars on its surface they call it CP. In chronic pyelonephritis one or both kidneys contain gross scars but even when involvement is bilateral the kidneys are not equally damaged. This uneven scarring is useful in differentiating chronic pyelonephritis from diseases that cause symmetrical contracted kidneys e.g. chronic glomerulonephritis . It is concluded that renal scarring after acute pyelonephritis in children is more common than has been previously thought. Although children with gross VUR and recurrent pyelonephritis are at the greatest risk renal scarring is more often seen without these risk factors. Pathologically there is atrophy and calyceal deformity with overlying parenchymal scarring. Chronic pyelonephritis may progress to chronic kidney disease. An esophageal perforation is a hole in the esophagus. The esophagus is the tube that food and liquids pass through on the way from your mouth to your stomach. The most common cause of esophageal perforation is instrumentation of the esophagus during endoscopy e.g. dilation of esophageal stricture or achalasia removal of foreign bodies or application of esophageal endoprostheses . The prevalence of perforation during diagnostic endoscopy is about 0.05 and 2.6 following instrumentation . Over the last few years endoscopists have been managing these patients with resection or ablation of lesions which puts patients at a higher risk for perforation. Perforations can also occur during dilation of tough esophageal strictures that develop after endoscopic management of Barrett esophagus. Core tip Patients with early esophageal cancer often experience endoscopic submucosal dissection with complications including bleeding perforation and stenosis. Although most cases are successfully treated conservatively perforation is a life-threatening complication and can require surgical intervention. This results in the leaking of food and sometimes even digestive fluid into the chest and it can result in a serious infection. The phrase chronic pyelonephritis CP is not really one diagnosis. It is meant to mean the long-term damage done by recurrent urine infection to the drainage system of the kidney. But it has come to be a fallback diagnosis i.e. when doctors are not sure of the cause of kidney failure and the kidney has scars on its surface they call it CP. In chronic pyelonephritis one or both kidneys contain gross scars but even when involvement is bilateral the kidneys are not equally damaged. This uneven scarring is useful in differentiating chronic pyelonephritis from diseases that cause symmetrical contracted kidneys e.g. chronic glomerulonephritis . It is concluded that renal scarring after acute pyelonephritis in children is more common than has been previously thought. Although children with gross VUR and recurrent pyelonephritis are at the greatest risk renal scarring is more often seen without these risk factors. Pathologically there is atrophy and calyceal deformity with overlying parenchymal scarring. Chronic pyelonephritis may progress to chronic kidney disease. Patients with chronic pyelonephritis may have residual foci of infection that may predispose to bacteremia or among kidney transplant patients seed the urinary tract and transplanted kidney.
